Voluntary Participation
I acknowledge that my participation as an Advisory Board Member for the Clinical Obesity and Research Network (CORN) is voluntary and based on my acceptance of the invitation.

Tenure Commitment
I agree to serve as an Advisory Board Member for a period of 2 years from the date of confirmation, unless otherwise mutually modified.

Roles & Responsibilities
I understand that my role may include (but is not limited to):
* Providing scientific and clinical guidance
* Participating in advisory meetings (virtual or in-person)
* Reviewing and contributing to academic or clinical documents
* Supporting CORN initiatives in education, research, and clinical programs

Use of Name, Designation & Photograph
I grant CORN the permission to use my name, designation, affiliation, and photograph in official materials, website listings, communications, and reports related to Advisory Board representation.

Confidentiality
I agree to maintain confidentiality regarding any non-public information, documents, or discussions shared with me in my capacity as an Advisory Board Member.

No Conflict of Interest
I confirm that I will disclose any potential conflict of interest arising during my tenure and act in accordance with professional ethics.

Non-Remunerative / Honorary Role
I understand that this is an honorary / non-remunerative advisory role unless specifically stated otherwise.

Data Usage Consent
I consent to CORN securely storing and using my submitted details for administrative, communication, and documentation purposes related to Advisory Board activities.

Right to Withdraw
I understand that I may withdraw from this role at any time by providing written notice to CORN. CORN also reserves the right to make changes in Advisory Board composition.
